On the North Wing of our hospital, we provide complex continuing care by offering services from a multidisciplinary care team to all individuals requiring rehabilitation, reactivation or palliative and chronic care. We also offer the necessary support to the family of our patients. Our commitment is based on continuous quality improvement with the maximum participation of the patient and his family. This team approach prioritizes client-centered care.

A multidisciplinary care team offers the services. Your stay with us depends entirely on you, since you are the best person:

  • to inform us of your condition
  • to take part in your treatments
  • to participate in your personal daily care
Since 1985, our on-going partnership with the Ottawa Rehabilitation Center brings supplementary expertise to our clientele.

How we operate

On the North Wing, we use a multidisciplinary team approach.

The team is composed of:

  • you, the client
  • a pharmacist
  • a physician
  • a speech therapist
  • a sector head
  • a discharge planner
  • a nurse in charge (during the day)
  • a palliative care coordinator
  • an occupational therapist
  • a psychiatric nurse
  • a physiotherapist
  • a psycho-geriatric nurse
  • a dietician
  • and several volunteers
Once a week, team members meet to discuss the status of each patient as well as the goal to reach. This is very beneficial for you as each member of the team offers ideas and suggestions to provide better coordination of your care and planning of your discharge. We assess and observe your abilities. Upon your arrival in the unit, we will start to help you resume as much of your personal care as possible.

When necessary, a case conference is planned for you and your family to inform and plan your care as well as plan your discharge.

  • If your medical condition requires therapy, a schedule will be established and you will be asked to comply to it fully.
  • You can still refuse this routine. However your decision to do so, could therefore initiate a premature discharge.

Role of the Occupational Therapist

The occupational therapist interacts with the patients to restart, improve and maintain significant activities of daily living. Depending on the degree to which a patient's physical and /or cognitive capabilities have been affected by an accident or an illness, the occupational therapist will implement a program adapted to the needs identified during the assessment as well as those identified by the patient and his/her family. Various interventions will be selected among the following:

  • stimulation and retraining of functional movements, for mobility and to look after one's personal care to reach a maximum level of autonomy and privacy;
  • therapeutic activities to reduce effects to the thought process, to memory, to visual and to communication functions;
  • prescription for a wheelchair or construction of special equipment meant to help with walking, to correct posture, to prevent contracture, sores and pain, to favor mobility and to help resume autonomous personal activities;
  • when needed, visits to the patient's home in order to make recommendations and modifications which are meant to make the return home easier, more efficient and the safest possible.
    For example:
    • making the home environment accessible;
    • ensuring that the bathroom installation is functional
    • ensuring there is sufficient space for the use of a walker or wheelchair, etc.

Telehealth Clinic: Evolution/consultation

A multimedia communication system (computer/video-camera/conference call) gives us the opportunity to have a direct contact with the Ottawa Rehabilitation Centre clinical team. The occupational therapist is responsible for the reference process, the on-line consultation and data processing system's operation. The clinic's multidisciplinary team who is specialized in rehabilitation offers consultation and evaluation services.

Reference to specialized services

Within the scope of specific objectives for rehabilitation, a request for specialized services and resources can be deemed necessary. If needed, the occupational therapist will prepare a referral for:

  • A driving evaluation program (Ottawa or Montreal)
  • The Ottawa Rehabilitation Center's orthosis, prosthesis laboratory
  • The Gatineau Rehabilitation Center's orthosis, prosthesis laboratory
